Evidence2Practice Heart Failure Tool

Evidence2Practice Ontario’s complimentary EMR-integrated heart failure tool offers a streamlined approach to support clinicians with appropriate assessment, diagnoses, and management, providing easier access to the necessary information at the point of care.

Features include:

  • Investigations into heart failure diagnosis
  • Medication plan management
  • Improved patient care planning & connection to self-management supports

Installation Steps:

All Accuro tools can be downloaded from the Accuro Form Cloud:

  • In the Form Editor, open the Tools menu.
  • Select the dropdown option Publish/Download.
  • Search for “E2P” to view the complete suite of forms.
  • Highlight and Download the following forms for Heart Failure:
    • E2P – HF – Heart Failure Management Tool
